FD-258 & fingerprint cards

FD-258 vs. FD-1164: Which Fingerprint Form Was Requested?

Check the form required for your specific request. DOJ identifies FD-258 as an Applicant Fingerprint Form; the FBI publishes FD-1164 for fingerprinting for Identity History Summary checks. Do not assume every FBI-related request requires only FD-258, or that either form can replace the other in every agency’s process. Neither completed card is itself an FBI report.

The recipient and route decide the useful question

Instead of asking which form is “better,” ask which one your recipient and submission route accept. Identify whether you are arranging a personal record request, responding to an employer or licensing authority, or ordering physical cards for another stated purpose.

Keep the original instructions with the form. A generic search for “FBI fingerprint card” may return several documents without explaining your own program’s requirements.

Check the label before the appointment

Look for the form number and any program-specific wording on the document you received. If the instructions name FD-258 but an attachment is labelled FD-1164, ask the sender to resolve the discrepancy. Do not silently switch one for the other because the fingerprint boxes look similar.

If no form is named, tell the provider the purpose and requesting organization. Ask for confirmation of the accepted form before capture, rather than ordering a standard card without explaining the task.

Agree what you are buying

For a Hallandale Beach appointment with Oraz Fingerprinting, distinguish the physical capture service from any later processing.

  • Which form and physical format will be used?
  • How many completed originals are needed?
  • Is the service cards only, or does an agreed report-support route follow?
  • What still requires confirmation from the recipient or processing partner?

A card does not complete the report or apostille

If your goal is a document for another country, explain that from the start. Capture, the eventual report and any further document certification should be discussed as separate stages. Having a named fingerprint form does not confirm eligibility for every electronic processing option or prove that a final report has been requested.

Questions about this topic

Is FD-1164 an alternative name for the FBI report?

No. It is a fingerprinting form for an Identity History Summary request, not the issued report.

Can I choose the form by color?

Do not rely on color or a familiar layout. Confirm the form number, format and recipient’s instructions.

Should I obtain both forms just in case?

Only if the confirmed request calls for both. First resolve the actual requirement so you do not pay for unnecessary capture or cards.
