How do I prepare for my fingerprinting appointment?
Open your CHAI screening profile, follow your licensing instructions and complete the Privacy Policy Affirmation. Bring the profession-specific ORI and required ID. Check existing retained fingerprints before paying for a new scan.
What else should I check?
Botox and medical aesthetics: use your underlying medical license, such as MD, DO, APRN or PA. There is no universal “Botox ORI.” EMTs, paramedics, pharmacy interns, registered pharmacy technicians and radiologic technologists have separate exceptions; military-spouse routes may differ.
Does your program ask for an ORI?
Identify the requesting program first. Use the directory as a reference and confirm the correct value with the organization before submission.

Is MQA a separate fingerprinting company?
No. MQA is a division of Florida DOH that handles professional regulation; a fingerprinting provider performs the capture and submission service.
Does renewal always require new fingerprints?
Not necessarily. Check retained-print status and whether DOH can use the existing screening for your application.
Will a personal FBI report replace the DOH check?
Do not substitute it. Follow the state licensing screening route identified for your profession.
